Target Age Groups Families with children aged 0-18 years.

Eligibility Criteria Children whose needs are identified as requiring a child welfare response through the social work initial assessment process.

Referrals Referrals are accepted from a wide variety of Sources including self-referrals, social workers, Hospital, GP’s, PHN’s and schools
